Studio Policies Ambition Performing Arts (APA) Monthly & Full Year Payments APA clients choosing to pay monthly will have the convenience of automatic monthly withdrawls by EFT (Electronic Funds Transfer) and must provide a VOID cheque at the time of registration. Online registrations can be completed with Visa or Mastercard as a method of payment for all sessional classes, or 1st month's and registration fee for non-sessional programs. All non-sessional clients choosing monthly payments must provide a VOID cheque prior to the first dance class to avoid penalties. Full Year Payments - by cash, debit, cheque, Mastercard or Visa. Monthly Payments – by EFT (Electronic Funds Transfer) only, a withdraw from your active bank account using a VOID cheque. An EFT form and Void cheque must be completed before being registered into the class of your choice if you wish to pay monthly. Costume Deposit Policies Recital Costume deposits will either be made with your full year payment OR you can authorize Ambition Performing Arts to withdraw recital costume deposits on the First business day of December from your EFT on file. **You may choose to pay this upfront in the office on or before November 1st , should you prefer not to have the direct withdrawal on December 1st. In order to help competition students manage their deposits, Competition Costume Deposits will be withdrawn November 1st. Deposits are as follows: **Please read through carefully, all sections are considered agreed to by the account holder when submitting online registrations** Registration Fee A non-refundable annual registration fee of $25.00 per student or $35.00 per family is due upon registration. Registration fee is payable by cash, debit, or cheques and is required at time of registration(payable to Ambition Performing Arts Inc.). Lesson Fees Fees are based on a 30 class season. Full year payments may be made by cash, cheque or debit. A 5% discount is available for full payment of the season in advance. Monthly payments will be made on the first business day of each month. Registration will not be confirmed or lessons provided until lesson fees have been submitted and registration fees have been paid. Please note that these fees do not include fees for extra classes, competitions and rehearsals, examinations, costumes or class attire. Non-payment or default on payments will result in a charge of 18% per annum. Returned Payments Returned Cheques, Declined Visa/MC, Rejected EFT’s - are subject to a service charge of $30.00 per-transaction. Priority Registration For your convenience, this contract will automatically renew itself for subsequent dance seasons. APA will continue to debit your method of payment on a monthly basis from October to June on the first business day of the month. You will receive your schedule and fees for the next term by April 15th. You may confirm your priority registration by paying your registration fee and first month's (September) tuition fee, either in the office or online. Costumes A costume deposit will be due on the 1st business day of December. This fee will be collected through your method of payment for your monthly fees. Full year payments will be required to pay this amount at the beginning of the year with registration. Competition costume deposits will be withdrawn on November 1st. 10 Month Program Cancellation Our studio must be advised of withdrawals or cancellations in writing 30 days in advance of the date of withdrawal. Payments that fall during that period must be remitted. Part months and missed classes will not be refunded. There will be no cancellations accepted from March 1st through to June 30th due to show preparations. Sessional Program Cancellations There will be no refunds issued for sessional programs. Refunds will only be made available should APA choose to cancel the registered sessions. Part sessions and missed classes will not be refunded. Summer camp cancellation must be submitted in writing 14 days prior to camp start date. A $50 administration fee will be withheld on all summer camp cancellations. Cancellations or withdrawals within 14 days of programs commencing will not be refunded any amounts. Photographs I understand that Ambition Performing Arts Inc. may use photographs of my child on the website (no names posted) and within its premises. I also understand that videotaping of my child may occur as part of lessons, rehearsals and shows. I grant Ambition Performing Arts Inc. permission to use these photos and videotaping as advertising and learning tools (without compensation). Waivers Please note that all dancers (or, if under age of 18, their Parent or Guardian) are required to sign this waiver. I am aware that the use of the studios and the participation in dance lessons involves inherent risks and hazards. I freely accept and fully assume all such risks, dangers, and hazards and the possibility of personal injury, death, property damage, or loss resulting from such risks and hazards. I voluntarily agree to release APA from any and all liability for any loss, damage, injury or expense that I or my next of kin, successors or dependants may suffer or incur as a result of my use of the APA facilities due to any cause whatsoever including negligence on the part of APA. As the Parent or Guardian of (Student name here) , if I cannot be contacted, I authorize Ambition Performing Arts Inc. to seek medical services in case of serious injury or illness. I further agree to accept financial responsibility in excess of the benefits allowed by my health plan. I understand that APA and its Directors will not assume responsibility for any lost or stolen property, or for any bodily or personal injury consisting of or arising out of any participant practicing in any physical training, athletic activity or contest.
